Anyone can make a referral.
Follow this simple 3-step process to refer a patient:
Step 1: Call 972-224-1876 or click here to start the referral process.
Step 2: Our Admission/Intake Coordinator will obtain all necessary
information and schedule a meeting with an Elysian Representative.
Step 3: Our Elysian Representative will visit with you and/or your
family to evaluate the case and initiate care, if eligible.
