Levels of Care
Elysian Hospice recognizes that everyone is unique and
requires different levels of care – we respond by delivering the right
level of care at the right time.
Routine Care: This level of care is offered wherever you
call home. Our team of professionals schedule regular home visits to
ensure that all your medical, social and emotional needs are met.
Respite Care: Occasionally, the primary caregiver needs a
break from providing ongoing care at home. During this time, a
short-term inpatient stay at a pre-qualified facility may be provided to
the patient in order to give the caregiver a rest. Elysian Hospice will
make arrangements when the need arises.
General Inpatient Care: When pain and symptoms at times
become unmanageable, the patient may be admitted for short-term
inpatient care. When the symptoms are stabilized, the patient can return
home.
Continuous Care: In the event a medical crisis arises and
cannot be managed by the caregiver, an assessment will be performed and
if necessary a recommendation will be made for continuous care. This
level of care is evaluated daily, and when the crisis is over, the
patient returns to routine care.
